One of our engineers is having a similar issue with slightly different error, running "arc install-certificate" would always fail with the following error:

@avivey If you follow the steps in my description, you should be able to reproduce the problem. I'm also attaching a screenshot which indicates that a maniphest was reopened from "Accepted" to "Resolved" twice because the merge commit msg also contains the original commit msg "... fix T...."
@avivey I updated description to include version info, repeatable and steps to reproduce, is there anything else you guys need?

@epriestley I see, tried the same steps and it doesn't play directly within chrome. Thanks for investigating.
@chad The first video still doesn't play in Chrome (Version 58.0.3029.110 (64-bit); seems working in FF and safari now.

@avivey I no longer run into this problem, please feel free to close this bug.
@epriestley not sure if the current phabricator install has been upgraded to lastest code that contains this fix yet, but the first video included in this bug which is a mov file still doesn't play. If I download it, it works, so the mov file itself is working.

@avivey Thanks! my php version is PHP 5.5.9, would you recommend upgrading? I didn't find anything in my php error log though, any suggestion where else I could look into to find out the cause?

Not sure if this is the right place to request for this, in an earlier build, the cards have username of the task owner on it, which is convenient to do a quick search using Ctrl+F to find tasks by a particular username. In the latest stable build, usernames on the cards are replaced with user photos instead, which makes Ctrl+F by username impossible. I understand that "Advanced Filter" is a workaround, but it is less convenient.
