Looks like I can't? Running from the command line it says that permission is denied, through the Web UI it's less informative or I don't know where to look: https://secure.phabricator.com/drydock/operation/106/

I signed the Corporate CLA, do I need to do the individual record or does this just need something outside of what I can see/do with secure.phabricator.com?
Great, and I agree it's a bit weird -- so can I get that patch to include the transactionID so I can hack it together?
Or is the idea that someone pastes T123#456 into chat, and you want to not only expand the T123 part, but also provide context about the #456 part? That is, this bot is reacting to human messages, not to events from Phabricator?

The problem we're facing now is that when we do have a link generated by Maniphest, we cannot use the API to reconcile what that is.

Ah, thanks! I think you are right, that is the problem.
Peculiar -- this behavior doesn't exhibit here, but I just did the same thing of removing a subscriber and using Cmd+Enter (screenshot of that: https://www.dropbox.com/s/qp8tyxrh7mq86xa/Screenshot%202016-02-02%2007.07.46.png?dl=0) and the action was not taken.
We're using 2015 W52 -- I checked the changelog since, didn't see anything that seemed relevant.

Is it known/expected that if a project has a "/" in the name (like "team/oncall") then the prefilling does not work?
