I have seen this issue a few times and never really worked out the cause until today. I have a bunch of tabs on my work computer that are long lasting. It seems that whenever I upgrade our Phabricator install (and in the processing, restart the Aphlict server), the Aphlict client SWFs crash ("Shockwave Flash has crashed").
