Improve the power of email settings

Description

  • Generalize the "mail tags" framework so things aren't hard-coded.
  • Provide a global "no email" option.
  • Provide a "no password resets" option.
  • Move formatting out of notification preferences and make way for an HTML option.
  • Allow users to choose "email", "notification" or "none" levels.

I marked D10241 as fixing this. I didn't do the disable-password-reset email thing; that might be nice eventually but hasn't actually become a problem yet.
