Unable to create new forms, even though I am an administrator

Description

Upgraded the Phabricator to the latest "stable" release.

Reproduction steps:

Click on Maniphesht.
Click on "Create Task"
Click on "Configure Form" button.
It takes you to the forms page, which would be of the url : https://phabricator.mydomain.com/transactions/editengine/maniphest.task/
Click on "Create Form" link.

Expected Result:
A new form would be displayed. Create new forms in earlier release.

Actual Result:
Seeing the error popup as shown in the attached screenshot.

Post the upgrade unable to create new forms for Maniphest, seeing the below error.

I can not reproduce this issue by following your reproduction steps. Here's what I see on this install:
