Seemingly repeating transaction causing many emails to be sent

I am pretty confused by this and looking for any insight into what could cause this issue.

I have a user who has been in our system for about 20 weeks now who has recently started spamming people with emails. The same email saying the diff has been updated / created is being sent over and over (about 20 times so far). This has translated to about 600 emails being sent so far.

The strange thing is that although I can see in the users profile that this is happening on the actually diff it says it's updating no new transactions are being created in the timeline. I did check all the transactions in the database and all seem normal to me. I only see one of each and don't see any funny timestamps or value for the new or old value.

To summarize this question how can transactions appear on a users timeline but not on the corresponding object that they are updating?

EDIT:

The only notable thing about this diff is it has a large binary file in it.

I did find this in the daemon log

Trying to update Task 8326062 (PhabricatorApplicationTransactionPublishWorker) after lease expiration!

This error only happened once however and I can see from his feed that it has shown up about ~5 times today.

Sorry, missed this originally -- this is probably T10968, which has a fairly straightforward resolution but doesn't come up very often.
