Make wiki creation dialog more user friendly

Description

It's a bit confusing now because you have to enter a path combined with a doc name (so am I supposed to enter "/My Page" or "/my_page"?). Entering a path is very technical. Why not 2 fields instead?

  • Parent doc path (by default the path of the current document you are looking at)
  • Pew doc name

Yeah, we have something nicer with slug creation on Projects (JS that converts the title).
