I haven't confirmed this myself, but I have noticed that sometimes someone will accept a commit but the status will not be changed to accept. (I'll attach a screenshot if I can find an example.)

Someone else observed that the problem is when someone comments and accepts at the same time. That at least, is consistent with what I saw.
