Missing avatar in Feed for Test Notifications

Description

The feed entry for test notification is missing the icon. I guess it should use default icon.

How to reproduce:

  1. log in
  2. go to http://secure.phabricator.com/settings/user/username/page/desktopnotifications/ (replace username with your username)
  3. click on Send Test Notification button
  4. go to homepage
  5. look at the feed (see image below)

We'll eventually not publish stories at all (i think there is another task, just can't find it). For now though this is working as intended.

I can't find the task. I'm like 98% sure @epriestley made one because someone kept spamming it on us.

I intend to remove them from feed, but it might have just been on a diff somewhere and not made it to a task.
