How to reproduce:
# create document (this creates 1st revision)
# delete document (this creates 2nd revision)
# delete document again (delete link is still present if document is deleted) and this creates 3rd revision
I think, that document is deleted, then it should be deleted for real OR at least do not show on `/w/` homepage.
