Add ability to secret questions and other password reset info along with password in Passphrase

Description

Passphrase is great to finally have a central place to store most credentials used by an org. Sometimes you need to store not only the login / password for a service but also any reset info like DOB or secret question / answer.

It's not practical to create a Passphrase record for each. Can we get a few extra encrypted fields or a generic "notes" one?

I think this is generally reasonable. See also T4721 for some similar improvements.

There's now a generic "note" credential type which lets you store a big chunk of secret text.
