I have tried:

Maniphest -> Create Task -> Configure Form -> View Form Configurations -> Create Form

But I am shown a form with three fiels: Name, Preamble, Visible To:

So I:

Enter form name -> Create new form

Now I am shown a preview of a form with: Name, Preamble, Visible to.

I can edit THIS form, but cannot access creating a new form with a starting point of "create task" for example.

So, how do I Create a new Task form?
