While using a self hosted instance of Phabricator ponder we faced an issue where anyone who answered a particular question (Q25) was blocked from answering any more answers with this error

#1062: Duplicateentry'0-PHID-USER-dugop5kyzbrhcfhh37q4'forkey'key_oneanswerperquestion

Answer Summary

I dug into the database and found out that there was an entry for all these users for a questionID=0. After I removed this from the database the users were able to answer questions. Not sure why this happened but would be helpful if this can be reported as a bug.
